Excel PDF Form Filler is a tool that generates the new CMS 1500 02/12 form with data populated from an excel spreadsheet.
When you select a row on the Excel spreadsheet and click on “Generate PDF”, it generates a pdf form populated with the data from the excel spreadsheet.
You can print multiple claims at once by selecting the columns you would like to print.

The pdf form has a functionality to turn the layer “on” or “off” so that you’re able to print with or without the CMS 1500 form. In other words, you can print data-only without the form if you would like.

    - What fields can it fill in the CMS form. Please provide the list
    - Can the excel file contain multiple rows? Does the tool generate multiple PDF documents if the excel file contain multiple rows?

      The tool fills in every field of the CMS 1500 form. And yes, the Excel file can have multiple columns for multiple claim data; you can generate multiple PDF claim documents from the Excel spreadsheet.   2. I want to maintain a database of claims to be printed and send each claim to your form to print continuously until each record has a printed claim form. Will your form accommodate this process?

      You will be able to import the claim data from your database to the Excel spreadsheet and then print all of the claims at once.
