  1. Few questions related to the excel to CMS1500 tool
    - What fields can it fill in the CMS form. Please provide the list
    - Can the excel file contain multiple rows? Does the tool generate multiple PDF documents if the excel file contain multiple rows?

    1. Hi Sunil,
The tool fills in every field of the CMS 1500 form. And yes, the Excel file can have multiple columns for multiple claim data; you can generate multiple PDF claim documents from the Excel spreadsheet. Here is a link to the Excel spreadsheet for the tool which lists all of the CMS 1500 fields. Excel Spreadsheet for the tool
Please let me know if you have any further questions.

  2. I want to maintain a database of claims to be printed and send each claim to your form to print continuously until each record has a printed claim form. Will your form accommodate this process?

    1. Hi Kevin,

You will be able to import the claim data from your database to the Excel spreadsheet and then print all of the claims at once.

    1. Hi Jon,
